Chevron Caltex Fuel Discount Data Sharing Consent Form - Diamond, Sapphire Tiers & Streaks

Dear Driver-Partner,

To enroll and enjoy the extended Chevron Caltex's 29% fuel discount, you will be required to fill up this consent form. Upon getting your consent, we will release your personal information to Chevron.

The submission of personal information would be strictly confidential, and would only be used for the purpose of enrollment into the Chevron Caltex Fuel Discount program.

After activation, you can proceed to download and register on the CaltexGo app to redeem the fuel benefits. All Caltex fuel benefits will be accessible via the CaltexGo app.

Please note that submissions after Monday will be processed next week, Wednesday.

Example:

Submissions on Monday will be processed Wednesday, on the same week.
Submissions from Tuesday - Sunday, will be processed Wednesday, next week.
