PSV Medical Check-Up is now available at GDC Axis.

Address: Level 2, Grab Driver Centre Axis (Location)

Time: 9 am - 5 pm (Monday - Friday) please note that we will be closed for lunch break from 12 pm to 1 pm on Monday - Thursday, and from 1:30 pm - 2:30 pm on Friday

Cost: RM50 (First Time), RM30 (Renewal), add-on RM20 (Blood Test for Diabetes)

Payment: GrabPay, Cash, Debit/Credit Card & E-wallet

IMPORTANT:
1. If you have sight issues (such as long sighted or short sighted), please make sure you bring your glasses.
2. If you have high blood pressure, please bring your medical report before doing the PSV Medical Check-Up.
3. If you have diabetes, please bring your HbA1c report before doing the PSV Medical Check-up.

4. For first time, please bring the JPJL8A form. Download here: grb.to/MCF

6. For new driver-partner, please sign up to be a Grab driver-partner before your appointment. Sign up here: Register Grab

Masa: 9 pagi - 5 petang (Isnin - Jumaat) Sila ambil perhatian bahawa klinik akan tutup untuk waktu rehat makan tengah hari dari 12 tengah hari hingga 1 petang pada hari Isnin - Khamis, dan dari 1:30 petang - 2:30 petang pada hari Jumaat

PENTING:
1. Jika anda mempunyai masalah penglihatan (seperti rabun jauh atau rabun dekat), sila pastikan anda membawa cermin mata anda.
2. Jika anda mempunyai tekanan darah tinggi, sila bawa laporan perubatan anda sebelum melakukan Pemeriksaan Perubatan PSV.
3. Jika anda menghidap diabetes, sila bawa laporan HbA1c anda sebelum melakukan Pemeriksaan Perubatan PSV.
